Mortgage interest and state taxes arrive as forms. Charitable giving arrives as a memory, and noncash giving arrives as a memory of a bag. That asymmetry is why the noncash line on Schedule A is simultaneously the most under-claimed number on the return and the one with the least evidence behind it.
Where the noncash line goes wrong
Round numbers with nothing behind them
"Goodwill, $400" is a claim with no method attached. Form 8283 asks for the method you used to determine fair market value, and a suspiciously round figure with no source is the weakest possible answer to that column.
The $5,000 line arrives by accumulation
Clothing is a group of similar items, so a year of drop-offs is tested together. A wardrobe cleared over twelve months can reach the appraisal threshold without any one donation looking large.
The acknowledgment has a deadline
A contemporaneous written acknowledgment must generally be obtained no later than the date you file the return for the year of the contribution. Chasing one after an audit letter is too late by definition.
Records die with the software
